Welcome to the HBO Reward Card Activation page. By visiting this page, you can activate the HBO Reward Card you received based on your sales performance as a Customer Service Representative for an HBO affiliate. In order to activate your HBO Reward Card, please enter your card and personal information below and click Submit. Once your information has been validated, you will be prompted to complete your activation on My Prepaid Center. Your home address and social security number are required for year-end 1099 reporting, should you meet the applicable tax threshold.
The personal information collected on this page will solely be used to activate and maintain your HBO Reward Card.
